TPAN supports community health with a comprehensive range of programs in Chicago and through our national publication, Positively Aware. Our staff is based in Edgewater, a north-side Chicago community adjacent to neighborhoods with the highest rates of new and existing HIV infections. We focus our services on residents in Chicago neighborhoods most affected by health disparities like poverty, unemployment, and chronic illness. As a community-based organization for over three decades, TPAN has become a one-stop shop for people living with and affected by HIV.

In March 2023, TPAN finalized an affiliated merger agreement with Vivent Health, an AIDS Service Organization celebrated for its exemplary track record in delivering comprehensive HIV healthcare and social services across four states. We believe TPAN’s partnership with Vivent Health heralds a new era ripe with potential for advancing integrated HIV care and social services in Chicago. Our aim is to tackle Chicago’s long-standing HIV health disparities head-on and help Illinois reach its Get to Zero goals to have zero new HIV transmissions and zero people living with HIV who are not engaged in care by 2030.

Central to our mission is to do all we can to help people living with HIV achieve viral suppression and reduce HIV transmissions in Chicago. By offering comprehensive and integrated access to HIV healthcare, medications, and social services within a single location, we aim to overcome the barriers that often deter individuals from seeking and maintaining care.
